Does Minoxidil Actually Work? What the Evidence Says

Minoxidil has been available as a hair loss treatment for decades. It was originally developed as an oral medication for high blood pressure, and hair growth was discovered as a side effect. From there, a topical version was developed, tested, and eventually approved by the FDA for use as an OTC hair regrowth treatment.

That is the origin story. But the more important question for most people is simpler: does it work?

What the evidence shows

Minoxidil is one of only two treatments recognised by the FDA as an OTC active ingredient for hair regrowth (the other being finasteride, which requires a prescription). This is not a marketing designation — it reflects a body of clinical evidence that the FDA evaluated before granting OTC status.

Studies on topical Minoxidil have consistently shown that it can support hair regrowth in people with androgenetic alopecia — the most common form of hair loss, sometimes called male or female pattern hair loss. Results vary significantly between individuals. Some users see meaningful regrowth. Others see maintenance of existing hair without further loss. A portion do not respond to treatment at all.

The evidence does not support using Minoxidil as a cure for hair loss. It supports using it as a treatment that, for many people, can slow or partially reverse the progression of hair loss when used consistently over time.

How Minoxidil works

The exact mechanism is not fully understood, but the leading explanation is that Minoxidil is a vasodilator — it widens blood vessels. When applied topically to the scalp, it increases blood flow to the follicles, which may prolong the active growth phase of the hair cycle and encourage dormant follicles to re-enter the growth phase.

It is worth noting that Minoxidil does not address the hormonal cause of androgenetic alopecia (dihydrotestosterone, or DHT). It creates a more favourable environment for the follicle to function — it does not change the underlying cause of follicle miniaturisation. This is why continued use is necessary to maintain results.

The 5% foam vs liquid

Both 5% liquid and foam formulations of Minoxidil have been studied. The 5% foam has become increasingly preferred for several reasons: it is easier to apply to specific areas, it dries faster, and it tends to cause less scalp irritation than the propylene glycol found in many liquid formulations.

The active ingredient — Minoxidil 5% — is identical in both. The delivery format differs, not the mechanism.

What it will not do

Minoxidil is not appropriate for all types of hair loss. It is indicated for use on the top of the scalp — not for receding hairlines, patchy loss, or sudden hair loss. It should not be used without knowing the reason for your hair loss. If your hair loss is sudden, patchy, or has no family history behind it, speak to a doctor before starting any treatment.

It will also not work for everyone. The label is clear on this, and it is worth taking seriously: results vary, and some people do not respond to Minoxidil treatment at all.

How long does it take to see results

Results are not immediate. Most users who respond to Minoxidil begin to see early signs of regrowth after 2 months of consistent twice-daily use. Some need at least 4 months before seeing any visible change. If no results are visible after 6 months of consistent use, it may be worth speaking to a doctor about alternative options.

The consistency requirement

Minoxidil only works if you use it. Twice daily, consistently, for as long as you want to maintain the results. If you stop, the hairs that grew as a result of the treatment will typically shed within 3 to 6 months — and you return to where you would have been without treatment.

This is not a flaw in the product. It is how the mechanism works. Minoxidil sustains a biological environment that supports hair growth. Remove that environment, and the follicles revert.
